ALL PAYROLL FORMS ARE DUE NO LATER than MIDNIGHT SUNDAY following the end of the pay period. See the pay schedule for due dates. You are always encouraged to submit your time earlier than the cutoff time so that it isn’t late and to allow time for DH and Dean approvals.
SUBSTITUTE FORM: Instructors/EW players should fill out sub. form when subbing out a class. DO NOT FILL OUT INVOICE OR ADDITIONAL HOURS FORM. If your sub has never worked for LACM, Please contact Human Resources before subbing out of class.
ADDITIONAL HOURS: This form is to be filled out for additional hours. I.E. – grading, class prep., etc. Your Department Head and Daniel Walker will need to approve the hours prior to Payroll processing.
SICK TIME/PTO FORM: This form is for submitting sick time or PTO. It does not replace the need for the sub form if you subbed out your class.
MEAL BREAK WAIVER FORM: If you work more than 5 hours, but less than 6 you can voluntarily waive your 30-minute unpaid meal break. If you work more than 6 hours straight, you cannot waive your break. Unpaid meal breaks must be taken prior to the beginning of your 6th hour working. The waiver should be submitted prior to your 5 hours of work.
